Forms

Applicants must bring three (3) completed copies of the “Request for Live Scan Service” form. This is provided by your employer or requesting agency. Please have all forms filled out prior to your appointment time.
 

ID

Valid photo identification is required. Expired or photocopied id’s will not be accepted.
 

Forms of ID

Acceptable forms of identification are

Drivers License, State ID Card, Green Card, Alien Registration Card, Passport
 

Fees

Department of Justice $32 set by State of California

Federal Bureau of Investigation $17 set by State of California

Rolling Service Home Office $23

Rolling Service Mobile $25

Travel fees quoted if needed.

All applications have different fee depending on the requesting agency.
